NOTE: This should be updated annually, and after any major health change. i.e., surgery, medication change, or injury, and after any change to contact information or address.
Medical Intake
Once you’ve filled out the online intake form above, please complete this intake within 4 hours of your scheduled appointment.
This is only necessary if you have specific medical issues that you need attention. If you’re using HSA/FSA, getting your sessions reimbursed, or are a VA Community Care Client, they may require this for coverage.
This gives us an idea of how you’re feeling TODAY, and allows us to document your conditions so you can submit your records for reimbursement. It’s essentially your progress notes.
